You need a special tool to replace the part that is screwed into the case. However, you dont necessarily need to replace that part at this stage. The pusher cap is held in with a long screw which goes through the middle of the pusher (the part screwed into the case).
I can pull out the top pusher the last 1mm with my fingers into the fully out position. The gasket around the pusher stem is likely what is keeping it from bouncing around freely. The only reason why it was noticed was because one of the pushers looked like it .
Take the watch to a reputable watchmaker (forget about the Omega boutiques) and ask them to take off the back, get a photo and post it here and while you're at it ask them to de-magnatise the watch (at 45secs it could just be that) and put the pusher back.
